The following is a list of items that must be received for the student athlete to participate with the Carolina Bearcats
• Registration form
• Liability release/insurance/non-refundability of fees
• Consent for treatment
Additional items that must be included with registration:
• Physical exam completed by physician (must have by August 1st)
• Picture ID: copy of drivers license, permit, school ID, passport, state issued ID (Player must not be 19 on or before August 31st)
• Proof of school: class ID, copy of schedule, etc.
• Football fee: $450. $150 deposit must be paid with registration. Payment plans available upon request.
